At the end of each shift, you will complete the task list. You must wait until the end of your shift to document tasks. You are required to leave a comment for any refusals of care and any cares/tasks that were not completed (i.e. client refused to shower, client independent, not needed). You must leave a 1-2 sentence comment with more detail about what was completed, how the client is doing, and if there has been any changes to their condition. This comment must be unique to the shift and can’t be repeated/the same sentence(s) every day. This is a state requirement! If you have a two-part shift, you must clock out and back in between segments. At the end of each shift, it is required that you get signatures for both you and your client. Please check that your clock out went through correctly! After you confirm your clock out your screen should turn bright orange and say “Good job, (your name)!” the shift will have a green line and the word “Completed” next to it on the “Shifts”/home page. If it gives you an error code message, hit “Dismiss” and click on the shift again. Click “Clock Out” and hit “next” until it lets you “Confirm” your clock out and you should see the orange screen. Documenting Tasks Per Idaho Medicaid rules, Personal Care Services (PCS), Attendant Services (A+D) and Homemaker Services (HMKR) are authorized and billed as distinct services. As a result, we have to separate the tasks for each service code as follows: Personal Care Services (PCS)/Attendant Care(A+D) Bathing, Dressing, Eating Meals (Assist), Medication Cues, Mobility, Personal Hygiene, Toileting and Transferring Homemaker (HMKR) Housework, Laundry, Meal Preparation, Shopping and Transportation *Companion Services include all tasks listed above, as does PCS if it is not attached to HMKR If your client’s shift is split into two different service codes/segments, you will have two different task lists as shown above. Each task lists must be completed during their designated time (example: the PCS task list must be completed during the PCS portion of the shift). You must complete the task list and clock out of the first part of the shift before clocking into the second part. You also must clock in and clock out at the correct times to ensure you do not go over the allotted hours for your client. We recommend you set an alarm for 5-10 minutes before the second part of your shift begins to remind yourself to complete your documentation. For most client schedules we have made the first part of your client’s shifts begin with either PCS or A+D service codes. Depending on your routine with your client you can choose to have your shift start with PCS/A+D or HMKR. If you normally come in and start with Homemaker tasks, you can choose to have your shift start with Homemaker so that is the first task list you complete.
